 IDENTIFICATION Before each signing all signers must be identified. All identification cards must be ORIGINAL, current or issued within the last 5 years, contain a photo, physical description, signature, and serial number. The following forms of identification are acceptable:

• State-issued driver’s license
• State-issued identification card
• U.S. military identification card
• Resident alien identification card (green card)
• U.S. passport
• Inmate ID issued on or after 1/1/91 by Utah Department of Corrections (DOC)for an inmate who is currently in custody.

Unacceptable Forms of ID May Include The Following:

• Birth certificate
• Social Security Card
• School IDs
• Credit Cards
• Debit Card
• No ID?
  This is a person who does not have, and cannot obtain, acceptable identification. This most often occurs when the person is an elderly person, a minor child, or a person with a disability. Utah law provides two additional methods of identification for these situations:
   
• The sworn written statement of a credible witness who is personally known to the notary and who personally knows the signer.
• The sworn written statement of two credible witnesses whose identities are proven to the notary and who personally know the signer.
